Did you contaminate the transmission fluid with incompatible implement fluid when you hooked them up? If so, that's your pulsating hydraulics and clutch problems. Purge the whole system, including the implements, change filters and re-fill.
The wheel coming off is just the guy with the wrench having too much to drink. Do drug and alcohol testing and the problem will clear up. It sounds like your operators need tightening too. Somebody would have to be 90% blind to miss a problem like that on the pre-start check.
